I, on behalf of each of myself, my spouse and my child/children (if any), hereby declare as follows:
- I (Insured)/We (Insured, insured spouse and insured child/children) hereby apply for AccidentSurance within the Hong Kong Special Administrative Region and declare that the statements and particulars given in this application are to the best of my/our knowledge and belief, true and complete, and that this application will form the basis of my/our contract with AXA General Insurance Hong Kong Limited.
- I/We further declare that:
- I am/We are in good health and free from physical impairment or deformity.
- I and my insured spouse are both under 60 years of age and the insured child/children is/are six months of age or over but under 18 years of age or is a/are full-time student(s) at a school, college or university under 23 years of age.
- I am/We are Hong Kong Special Administrative Region residents.
- I/We have never been denied personal accident insurance.
- I/We do not have any in-force AccidentSurance policy with AXA General Insurance Hong Kong Limited that was effected after 27 October 2002, and have not had any AccidentSurance policy cancelled during the last six months.
- I/We understand that insured person(s) who engage in work or activities listed in the Hazardous Activities List, will receive no benefits under the policy for Accidental Injury, Death or Medical Expenses which result from or are caused directly or indirectly by an Accident whilst carrying out such work or activity.
- I/We have read and understood the Personal Information Collection Statement, Terms and Conditions and the Policy Wording of this AccidentSurance and agree to be bound by the same.

Premiums may be paid by direct debit to a designated credit card account.

AccidentSurance will be renewed upon payment of premium when due, otherwise it will be cancelled automatically.
- Refund of Premium

This policy is subject to minimum payment of two months' premium in advance which is not refundable upon termination unless the policy is cancelled within 30 days of the receipt of the policy (i.e. from the date of application). - For annual payment policy
This policy is subject to the payment of the annual premium in advance. All premium paid will at all times be subject to the Premium Refund Rules upon the termination of AccidentSurance except where AccidentSurance is cancelled within 30 days of the receipt of the policy (i.e. from the date of application),where a full refund will be made.

Cancer Care Insurance Plan
Terms and Conditions for Cancer Care Insurance Plan
- Eligibility
The following persons are eligible to be covered:- Persons who legally reside in the Hong Kong SAR with Hong Kong identity card;
- Persons whose age from 14 days to 60 years old (inclusive) on the date of first time application. Cover may be renewed up to 79 years old.
- Premium Payment
- Premium must be paid by direct debit from HSBC credit card / account.
- Premium must be paid by direct debit from HSBC credit card / account.
- Renewal Arrangement
- A renewal notice will be sent to you 45 days before the policy anniversary.
- If you decide not to renew the policy, you will need to inform us at least 30 days before the policy anniversary, or the policy will be renewed automatically for another year subject to the payment of the renewal premiums before the due date as determined by us.
- Policy Cancellation
- You may apply for policy termination by giving a written notice to AXA at least 30 days before the anniversary of your policy. No refund of premium will be made once your policy is effected.
- AXA shall have the right to terminate the policy, or to revise the terms and conditions of the policy if you failed to act in utmost good faith.
- AXA will give you a written notice at least 7 days before such termination or revision. For such termination, you shall be entitled to a pro-rata refund of the premium paid provided that no claims have been paid or are payable under your Policy.

Fire Insurance
Declaration for Fire Insurance
- I (Applicant) have read and understood the Personal Information Collection Statement.
- I hereby apply for Fire Insurance within the Hong Kong Special Administrative Region and declare that the statements and particulars given in this application are to the best of my knowledge and belief, true and complete, and this application will form the basis of my contract with AXA General Insurance Hong Kong Limited. I further declare that:
- My insured property is built of bricks, stone and concrete and roofed with concrete.
- My insured property is less than 40 years of age.
- My insured property is not used for commercial purposes and is located in a permanent building in Hong Kong SAR.
- I am/will be the legal owner of this insured property by the Policy Commencement Date.
- I have never had any fire insurance applications rejected or accepted on special terms by any insurance company.
- I do not have any Fire Insurance policy of my insured property with AXA General Insurance Hong Kong Limited cancelled during the last six months.
- I understand that, in respect my choice of sum insured, this Policy Wording is subject to Average clause (refer to clause 11 of the policy) and Reinstatement Value Insurance (refer to clause A19 of the policy) in the event of under-insurance

Declaration for ResidenceSurance
- I, on behalf of each of myself and the joint applicant (if any), hereby apply for ResidenceSurance within the Hong Kong Special Administrative Region and declare that the statements and particulars given in this application are to the best of my/our knowledge and belief, true and complete, and that this application will form the basis of my/our contract with AXA General Insurance Hong Kong Limited.
I/We further declare that:
- My/Our home is built of brick, stone and concrete and roofed with concrete. The floor area stated in this application has included saleable area, terrace, forecourt, backyard and/or roof of the Home.
- In the past 3 years, I/We have never had any of my/our home, personal belongings or domestic helper insurance policy cancelled, declined, refused to renew or had special terms and conditions imposed upon a policy, or had accidental loss of or damage to home property, or had reported home insurance claims exceeding HKD 50,000.
- My/Our home is in a permanent residential building which is already constructed and less than 51 years of age, or will be constructed by the Policy Effective Date.
- I/We do not have any in-force ResidenceSurance or HomeSurance Policy with AXA General Insurance Hong Kong Limited for this home.
- I/We do not have any address or residence in Japan.
- My/Our home is built of brick, stone and concrete and roofed with concrete. The floor area stated in this application has included saleable area, terrace, forecourt, backyard and/or roof of the Home.

Terms and Conditions for ResidenceSurance
- ResidenceSurance is specially designed solely for domestic use in residential building, house, apartment or flat being situated in the Hong Kong Special Administrative Region (Hong Kong SAR). All structure of the home must be under legal construction.
- Premium Payment Method
Premiums may be paid by direct debit to a designated credit card account.
- Basis of Coverage
This insurance coverage for Household Content and Worldwide All Risks cover is based on 'New For Old'; that is, repairing the damaged property or replacing the property if stolen or beyond repair with a new article of the same kind which is of similar but not better quality.
- The successful applicant and his/her family members (including spouse, children, parents and other relatives permanently living with you in the home) must comply with all statutory obligations and take all reasonable steps to prevent loss, damage or injury; and maintain the home in efficient condition and good repair.
- The successful applicant must advise us of any change in his/her occupation of the premises, any permanent move of the home or circumstance which would increase the possibility of loss or damage. Premiums may be adjusted accordingly.

- I/We have not had, during the last four years, any illness, injury, ailment or condition requiring in-patient treatment or consultation with a specialist.
- I/We do not have any foreseeable need for treatment or consultation with any medical practitioner.
- I am, and the insured spouse (if any) is, under 60 years of age and the insured child/children is/are between six months and 21 years old inclusive or is a/are a full-time student(s) at a school, college or university under 23 years of age.
- I am/We are residents of the Hong Kong Special Administrative Region.
- I/We have never been denied personal accident or medical or hospital cash insurance.
- I/We do not have any in-force HospitalSurance or Refundable HospitalSurance policy with AXA General Insurance Hong Kong Limited, and have not had any HospitalSurance policy cancelled during the last six months.

Declaration for TravelSurance
- I hereby declare that I have been duly authorised by each of the persons covered under this application (together, the “Insured Persons” and each an “Insured Person”), including parent or Legal Guardian of the Child(ren) mentioned in this application form, to apply for TravelSurance and to make the following declarations for and on his/her/their behalf. I also hereby declare that each of the Insured Persons including parent or Legal Guardian of the Child(ren) mentioned in this application form has agreed to the information under this application including under these Declarations, and that it is a condition precedent to obtaining coverage for each such person that such Insured Person including parent or Legal Guardian of the Child(ren) mentioned in this application form has agreed to all such information. (This declaration is inapplicable to an Applicant applying for his/her own insurance only.)
- I, and on behalf of each of the Insured Persons, including parent or Legal Guardian of the Child(ren) mentioned in this application form, hereby apply for TravelSurance, deemed and accepted to constitute separate insurance in respect of each such Insured Person declare that the statements and particulars given in this application are to the best of the knowledge and belief of each of the Insured Persons, including parent or Legal Guardian of the Child(ren) mentioned in this application form, true and complete and that this application will form the basis of the contract of insurance underwritten by AXA General Insurance Hong Kong Limited (“AXA”) covering each Insured Person. I/Each of the Insured Persons, including parent or Legal Guardian of the Child(ren) mentioned in this application form understand(s) and agree(s) that no insurance will be effected until the application is approved and the premium and levy are received by AXA.
- I, and on behalf of the Insured Person(s), including parent or Legal Guardian of the Child(ren) mentioned in this application form, confirm that (i) all insured Adult(s) are aged 18 or above; (ii) all insured Children are under 18 years old; and (iii) all persons described in (i) and (ii) above are either residents of the Hong Kong Special Administrative Region or that they hold valid passports and have never been denied travel insurance.
- I, and on behalf of the Insured Person(s), including parent or Legal Guardian of the Child(ren) mentioned in this application form, hereby acknowledge and warrant that I will not/None of the Insured Person(s) will be travelling contrary to the advice of any medical practitioner or travelling in order to receive medical treatment. I do not know/None of the Insured Persons knows of any condition, cause or circumstance existing that may necessitate the cancellation or curtailment of the planned trip.
- I, and on behalf of the Insured Person(s), including parent or Legal Guardian of the Child(ren) mentioned in this application form, hereby declare that I/ We do not have any address or residence in Japan.
- I, and on behalf of the Insured Person(s), including parent or Legal Guardian of the Child(ren) mentioned in this application form, hereby acknowledge that AXA may contact me for the purpose of this application.
- I/We agree AXA General Insurance Hong Kong Limited (“AXA”) will use my/our personal details such as corresponding address, email address or mobile number to send me/us Policy-related information and documents by mail or merely by electronic means (such as by email or SMS) at AXA's discretion.
- I/We consent to The Hongkong and Shanghai Banking Corporation Limited (“HSBC”), as an insurance agent of AXA General Insurance Hong Kong Limited (“AXA”), using and transferring to AXA any information relating to myself/ourselves collected by HSBC from time to time in the course of or in connection with performing its obligations and duties as insurance agent for the purposes as set out in AXA’s Personal Information Collection Statement and other directly related purposes.
- I/We understand that the remuneration for HSBC sales staff is determined based on the staff’s overall performance with reference to a wide range of factors, and is subject to review from time to time, for the purpose of encouraging the building of deep, long-lasting and mutually valuable relationships with customers. It is not determined solely on financial performance.
- I HEREBY DECLARE that I understand that AXA may deduct any outstanding amount applicable to the Policy from sum received by AXA under the Policy/Policy according to the applicable statutory and/or regulatory requirement(s), including but not limited to levy collected by the Insurance Authority.
I and each of the Insured Persons have read and understood the Personal Information Collection Statement, Terms and Conditions (see below section) and the Policy Wording of this TravelSurance and agree to be bound by the same.
Terms and Conditions for TravelSurance
- All trips under TravelSurance must start from the Hong Kong SAR only.
- Period of Insurance
- For Single Trip TravelSurance, duration of the period should not be longer than 366 days.
- For MultiTrip TravelSurance Worldwide Standard Plan and Worldwide Basic Plan, the coverage will cease 100 days from the beginning of each trip.
- For MultiTrip TravelSurance Greater Bay Area Plan, the coverage will cease 15 days from the beginning of each trip.
- Eligibility (age limits)
- No age limit for both Single Trip and MultiTrip TravelSurance applications. MultiTrip TravelSurance is lifetime renewable.
- Adult means person aged 18 or above during the period of insurance.
- Child means person aged under 18 during the period of insurance.
- Premium Payment Method
- Premiums may only be paid by debiting from designated HSBC credit card account or HSBC bank account.
- For MultiTrip TravelSurance Policy, it will be renewed automatically with required premiums to be debited from the designated HSBC credit card account or HSBC bank account on an annual basis.
- Extension of Period of Insurance (Applicable to Single Trip TravelSurance only)
For extension of Period of Insurance after the Policy is issued, please contact AXA Insurance Service Hotline (852) 2867 8678 or visit any HSBC branch for arrangement before the expiry of Policy. The minimum additional premium is HKD30. - Application for the China Medical Card (applicable to MultiTrip TravelSurance Worldwide Standard Plan and Worldwide Basic Plan)
Only applicable for online application: if any of the Insured Persons under the application requires any coverage by the China Medical Card, all other Insured Persons (if any) under same application will be assumed to apply for such cards. If it is not your intended arrangement to have all Insured Persons to apply for the China Medical Card, please contact AXA Insurance Service Hotline (852) 2867 8678 or visit our branches for alternative arrangement. China Medical Card is already included for MultiTrip Greater Bay Area Plan and an optional cover for MultiTrip Worldwide Plans. - Cancellation

Upon the issuance of any Travel Alert for the planned destination, the Applicant may give notice in writing to AXA to cancel the Policy before commencement of the scheduled trip. Provided no claim has been paid, the Applicant will be entitled to a full refund of premium and levy paid. Under any other circumstances, no premium and levy will be refunded once the application has been accepted. - MultiTrip TravelSurance
The Applicant may give 7 days’ prior written notice to AXA to cancel this Policy to obtain a pro-rata refund for the premium paid for the unused portion of the premium (for the period of this Policy is not in force) provided that no claim has been made/will be made during the current Period of Insurance, such pro-rata refund of premium will be calculated on a pro-rata basis and subject to a minimum of 50% of the annual premium being paid has to be borne by the Applicant. - China Medical Card
Once the application is approved, the China Medical Card may be cancelled by giving written notice to AXA or by cancelling the MultiTrip TravelSurance Policy (for Wordwide Plan only). Such cancellation shall be effective when the notice is received by AXA or on the cancellation date of the Policy. No refund of premium and levy will be made once the cover is effected.

Motor Insurance
Declaration for Motor Insurance
I (who is the proposed policyholder), on behalf of myself and each of the named driver (if any), hereby apply for Motor Insurance within the Hong Kong Special Administrative Region and declare that the statements and particulars given in this application are to the best of my/our knowledge and belief, true and complete, and that this application will form the basis of my/our contract with AXA General Insurance Hong Kong Limited.
I/We declare and agree that:
- I and / or each named driver(s) have not been convicted for any careless driving, dangerous driving, drunk driving, drug driving or penalized with driving license suspension in the past 3 years.
- I and / or each named driver(s) have not accumulated over 12 driving-offence points cumulatively in respect of any traffic offences committed in the past 24 months.
- I and / or each named driver(s) have been holding valid driving license for 2 years or more.
- I and / or each named driver(s) do not have any motor policy being terminated or refused for quotation by another insurer in the past.
- My car is not used for tuition purpose, rental, leasing, hire and reward, or as a courtesy car.
- My car has not been modified from the manufacturer's standard specifications.
- My car is not used for rallies, competitions, trials, or motor sport.
- All information and answers to all questions, whether or not input by me, are true and complete in every respect.
- I understand that the liability of AXA General Insurance Hong Kong Limited ("AXA") does not commence until this application has been accepted by AXA and payment of the premium is made to AXA.
- I confirm that I am not acting on behalf of any other person for this application unless otherwise expressly indicated in this application.
- All information, answers to all questions and declaration, together with this application, shall form the basis and become a part of the policy.
- I do not have any address or residence in Japan.
- I must be the holder of credit card used to make payment for this application, and the credit card must be with HSBC.
- I will provide the Vehicle Registration Number to AXA within 14 days of the Issuance of Certificate of Insurance if the Vehicle Registration Number is not available at the time of issuance.
Terms and Conditions for Motor Insurance
- Premium Payment Method
Premiums must be paid by a designated HSBC credit card account. - The successful applicant (the Policyholder) must provide the Vehicle Registration Number of the insured car to AXA within 14 days of the Issuance of Certificate of Insurance if it is not available at the time of issuance.
- The Policyholder and the HSBC credit card cardholder of the premium payment must be the same person.
- Basis of Coverage
Under all circumstances, indemnity to the insured car shall be based on reasonable Market Value at the time of loss or damage. Market Value means the cost of replacing the insured car with one of the same make and model, of similar condition, specification and age as prevailing immediately before the accident. - The successful applicant (the Policyholder) and all insured driver(s) must comply with all statutory obligations and take all reasonable steps to prevent loss, damage or injury; and maintain the insured car in efficient condition and good repair.
- The successful applicant (the Policyholder) must advise AXA of any change on the insured driver(s) or insured car, or any circumstance which would increase the possibility of loss or damage. Premiums may be adjusted by AXA accordingly.
- Policy Renewal
About 1 month before the policy expires, the successful applicant (the Policyholder) will receive a renewal invitation from AXA by email. Policyholder has to confirm the renewal via this email and pay the renewal premium by HSBC credit card before the policy expires to ensure continuous coverage. - Policy Cancellation
AXA may cancel the Policy by giving seven days' notice by email to the Policyholder at his/her last known email address or by registered letter to the policyholder at his/her last known address.
The successful applicant (the Policyholder) may cancel the Policy at any time by giving seven days' notice to AXA provided no claim has arisen during the current Period of Insurance, and the Current Certificate of Insurance has been returned to AXA on or before the date of cancellation
The successful applicant (the Policyholder) shall be entitled to a pro-rata refund of premium for the period the Policy is not in force subject to a minimum non refundable premium of amount HKD500 (not including M.I.B. surcharge) for policy cancellation. - Privacy and Security Statement
